Your First Visit

We are looking forward to meeting and helping you. At your first visit our staff will ask you for information that we will need to take care of you. Be assured that this information will be kept confidential.

  • Personal Information Sheet
    It will be essential for you to fill out the Personal Information Sheet at your first appointment. If you have access to a computer and printer you may download the document and bring in the completed form to your first visit. See Patient Information Record.
  • Minors
    (patients under the age of 18) at the first visit must be accompanied by a parent or legal guardian ( with proper documentation of guardianship).
  • What to bring:
    • Referral - If your insurance company requires a referral, please make sure this referral has been made prior to your appointment.
    • Your insurance card(s)
    • A photo ID( identification)
    • A list of current medications
    • A list of medications previously taken for your allergy or asthma problem
    • If possible, previous allergy skin tests and any lab or x-ray reports that may relate to your current problem.
  • Insurance

  • What to expect on the first visit
    • On your first visit, the nursing staff will obtain your vital statistics (blood pressure, height, weight) and breathing tests may be administered.
    • Your physician will then take a complete medical and environmental history and do a physical exam.
    • Allergy testing may be done to diagnose & confirm any existing problems.
    • Laboratory tests on the problem may also be ordered.
  • We ask that you stop taking all anti-allergy medication (antihistamines or decongestants) for 5-7 days before testing to insure more accurate test results. See list of medications not to take.

After the results are read from the skin tests, as well as breathing tests & lab-blood tests, the allergist will correlate the test results with the patient’s history of allergy-like symptoms.

Discussion of the potential treatment plan and the ordering of any additional tests will be done at this time.
